This form is for students who are new to Faith & Grace Christian Academy or who did not attend during the last school year.

Students who attended Faith & Grace last year should complete the Returning Student Registration form instead.

Please fill out one registration form for each new student (K-12th Grade). Allow approximately 30 minutes to complete.

Please note: Both parents must submit an explanation of their relationship with Jesus Christ and provide information for three non-related references (we recommend preparing these ahead of time).
